This rebate form is not an order request. To qualify for the rebate, you must submit the form before the deadline. You'll receive a personalized rebate code via email to give to your preferred retailer when you place your order. Tell your retailer to reference your custom rebate code when they place your order with Suterra.
Disclaimer: This rebate is only valid on purchases of the CheckMate® Puffer® NOW Ace™ product (the “Eligible Product”) with a minimum aggregate net order quantity of 12 units or more. Rebate Form Deadline December 31, 2025 (“Rebate Form Deadline”). Rebates will be paid on net purchases of the Eligible Product. All purchases will be confirmed by Suterra with each authorized retailer. Actual and final net purchases shall be calculated by Suterra after adjustments for any product returns, cancellations, refunds or credits. Rebates shall be due and payable by October 31, 2026.
Details for Retailers: The Rebate Qualification Form deadline is not the deadline for order placement, but you are encouraged to place the grower's order as soon as you receive their request with a delayed delivery date based on the grower's desired deployment timeline. Please order hooks as necessary and note the estimated lead times in the Online Order Tool. Multiple Rebate Codes can be entered on a single sales order.
